But back to the original post, yes giving change broken out is SOP for tip laying purposes. If you didn't leave the five, you're a cheap little pecker.
40%? I always want to wait on your table! Well, I would anyway, just because you're you but the 40% would definitely not be a disinsentive. If 40% is the cutoff for not being a cheap little pecker, put me down with the cheap little peckers.
I am with you, there. That's insane. On $12.46 (after tax, mind you), they would have gotten probably the change and then $2.

Apparently the facetiousness of RUIT's post was lost somewhere along the line....

I'm sorry, he said she was cute. Lay the fiver. If YOU had my table, you would get two-fiddy added to a credit card sale.

Bingo. Since the $20 was enough to cover the entire bill, she probably thought the 3 singles implied "keep the change."

Do you still have pictures of the Pork-Chop Necklace your momma made you so the dog would actually play with you?

We had a turtle. And the dead flies kept crumbling when she tried to string them.

Hey now, what's it gonna be. He is either a 1% or a 47% don't throw another percentage in there.

FWIW, I am not a server, but I do run the register in a restaurant rather frequently, and when a server doesn't have change in their book, they'll come and break a bigger bill down with me. It is true that the five and five ones is somewhat SOP for servers, making it easier for guests to leave a tip, but also, it's more than entirely possible that the server didn't have a $10 bill, and that there wasn't one in the drawer at that exact moment. As for teaching simple math... you did get the correct amount back, just not in the denominational breakdown you were expecting.
